Terms of Service.
Overview.
These Terms of Service govern your use of prologuemd.com and the telehealth medical services provided by Prologue MD (“Prologue,” “we,” “us,” or “our”). By using our website or services, you agree to these terms. Please read them carefully.
If you do not agree to these Terms, you must not access or use our services. We reserve the right to modify these Terms at any time, and your continued use of the services following any changes constitutes your acceptance of the revised Terms.
Eligibility.
You must be at least 18 years of age and located in a state where Prologue’s telehealth services are available. Our services are not intended for use by individuals under 18.
By using our services, you represent and warrant that you meet these eligibility requirements. We may request proof of age or location at any time to verify your eligibility.
Our services.
Prologue provides telehealth medical consultations, prescription management, and medication delivery through partnerships with licensed physicians and accredited pharmacies. All prescriptions are written by licensed physicians based on their independent clinical judgment.
The services provided through Prologue are not intended to replace your primary care physician. We strongly encourage you to maintain an ongoing relationship with a primary care provider and to communicate with them about the care you receive through our platform.
Subscriptions & billing.
Prologue services are provided on a monthly subscription basis. Your subscription will automatically renew each month unless you cancel. You may cancel or pause your subscription at any time. Cancellation takes effect at the end of your current billing period. No refunds are provided for partial months.
Nothing is charged until a physician approves your plan.
No guarantee of prescription.
Submitting a clinical intake does not guarantee that a prescription will be written. All prescribing decisions are made by licensed physicians based on their independent clinical judgment. If a physician determines that a requested treatment is not appropriate for you, you will not be charged.
Medical disclaimer.
Prologue provides telehealth medical services and is not a replacement for your primary care physician or emergency medical services. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
Some medications prescribed through Prologue may be prescribed off-label — meaning they are FDA-approved for one indication but prescribed by your physician for a different purpose based on clinical evidence. Your physician will discuss this with you during your evaluation.
Limitation of liability.
Prologue and its partners provide medical services in good faith based on available clinical evidence. We do not guarantee specific health outcomes. Individual results vary. To the fullest extent permitted by law, Prologue shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services.
