Protocol № 03 · Hormonal

Testosterone Therapy.

A weekly protocol of testosterone and anastrozole, prescribed after a video consultation and confirmed low testosterone.

Testosterone declines roughly one to two percent a year after thirty. A physician licensed in your state confirms whether replacement is clinically appropriate and prescribes the protocol. Both medications are compounded by a licensed pharmacy partner and shipped to your door.

Testosterone cypionate restores serum testosterone toward the physician-determined target range. Anastrozole is an aromatase inhibitor that manages testosterone-to-estrogen conversion when clinically indicated. Testosterone is a self-administered subcutaneous injection (meaning just under the skin, not into a muscle) using a small insulin-style needle, given weekly into a fatty layer of the abdomen or thigh. Anastrozole is oral, taken on the schedule your physician sets.

Testosterone therapy supplies testosterone from outside the body. That suppresses the pituitary signal that keeps the testicles producing testosterone on their own, which can reduce testicular size and affect fertility during treatment. Gonadorelin is a compounded medication that keeps that pituitary signaling active.

Men who want to maintain testicular function while on therapy sometimes add it, and it is a common reason fertility comes up in the consultation. It sits outside the core protocol. Your physician will bring it up if it fits your goals.

Gonadorelin is a GnRH analog, self-administered as a small subcutaneous injection on a schedule your physician sets. It is prescribed off-label and billed at member rates.

Testosterone replacement therapy is one of the most studied hormone therapies in adult men with documented low or low-normal testosterone. Published studies have examined effects on body composition, bone density, energy, mood, and sexual function in men receiving TRT under physician supervision (Bhasin et al., Endocrine Society Clinical Practice Guidelines, 2018). Treatment requires regular lab monitoring of testosterone, estradiol, hematocrit, and PSA.

Testosterone Therapy is for adult men whose lab work confirms low or low-normal testosterone and whose physician determines replacement is clinically appropriate. Treatment begins after a video consultation and physician sign-off. Your physician will review your lab results, medical history, and goals before prescribing.

Testosterone Therapy is available to members at member rates. The medications run $124 per month, and with Practice Membership at $129, that comes to $253 per month. For comparison, concierge longevity practices providing similar physician oversight can run $5,000 to $15,000 per year.

Practice Membership covers the four quarterly sync video consultations required for testosterone monitoring. No additional fee for Schedule III follow-ups.

Nothing is charged when you join. Billing begins only after a physician has approved your plan.

Testosterone is FDA-approved for replacement therapy in men with hypogonadism. Anastrozole is FDA-approved for breast cancer treatment; Prologue prescribes it off-label as an aromatase inhibitor for testosterone-to-estrogen conversion management when clinically indicated. Both medications are compounded by a licensed pharmacy partner and tested for potency, sterility, and endotoxin levels before shipment. Gonadorelin, when added, is prescribed off-label to support testicular function during therapy and is compounded by the same pharmacy partner. Testosterone is a Schedule III controlled substance and requires an initial video consultation with your physician before treatment begins. Treatment is not appropriate for every patient, and outcomes vary. Talk with your physician about whether this protocol is right for you.
